The conversion add details to the lower pyramids levels and the city, which were just outlined in the original B4. It fleshed out also adventures seeds on the city level.

I was going to look over the Lost City map, and the Goblin Caves. All I can find is a side view. Wasn't there a detailed map of the areas below the pyramid ?

I'll note I recall Warrior's of the Eternal Sun's pyramid for the Azcans and the Shadow Elves (map design suggests pyramid-like).

If you still haven't got your hands on it, Paleologos reviewed it and gave a pretty good run down. I had a chance to read it a few years back and I believe it's the only effort that really fills out everything that was hinted at in the original module. On the downside, from what I remember the expansion only has 5E stats so you have to convert those.
The best fan expansion of B4 I've seen is Geoff Gander's Dungeon Master's Guide to Cynidicea, which now has a PDF, but it's still mostly background rather than making the city and areas around it ready to run. There are a few well fleshed out locations though and some scenario ideas.
Personally, while the upper pyramid that's fully detailed in B4 was a nice dungeon, and I loved the Cynidiceans with their factions and drug fuelled delusions, I never liked the idea of humans living in a city in a giant cave under the desert. I also knew I was never going to write up the city the way I'd want to.
So I took the pyramid and Cynidiceans and added them to the tropical crater city of I1 Dwellers of the Forbidden City. The Cynidiceans are the original inhabitants, controlled by a priest class who keep them supplied with drugs sourced from the surrounding jungle, and ruled by their noble class transformed into Yuan-Ti by demon worship. The mongrelmen are Cynidiceans in bestial masks acting weird under the influence. The B4 pyramid levels 1-4 is the tomb of a past king where the Cynidiean resistance groups meet. Level 5 is a temple to Zargon elsewhere in the city. The latest version of Dwellers of the Lost and Forbidden City is on my website.
There's also the thread I started here on new Cynidicean delusions. Just occurred to me I should add what I posted there to my PDF.
